Wolf and deer is a strange mix, so a deer skull with wolf teeth with the snout stretched further than a normal deer skull. Antlers are kept in. Deer-wendigos are usually depicted with deer ears too, so either they could have a mix of both ears, one or the other, or possibly both. Again for convenience sake, let's say it gets both, so it has 4 ears.
Finally the body, up to the neck it will have a large amount of hair on the scruff down to the shoulders, while the body itself will be rotten and deteriorating with the lich element mixed in. Body structure is taller than the wendigo with arms reaching down to its feet, but more muscle mass with the werewolf mix as well as a thicker skeletal structure, at least with the torso. Also, the ribcage and sternum exposed to the air with the wendigo mix, and finally the arms and head being stained with blood. Werewolf mix will make it both physically stronger and faster, making it that much more of a threat. With the lich, it will be able to use the bones of its victims to create an undead army, while also possibly having access to spells.
So what we're looking at here is a 4.5 meter tall or taller rotting hulkster of a monster, with a wolf-deer mix of a skull, 4 ears, muscular as fuck frame that can break down trees and probably chase a car down and constantly rotting flesh it tries to replace with human flesh, all the while commanding an army of the dead, having access to spells, and being under a beserk rage that will never end until it is finally dead, which will be hard to do because of being immortal until it's phylactery is destroyed, which I doubt people will know to find while dealing with this fuckfest.
