Emily and Marten have canonically been told so, and Marten has even seen her nude. No one is under the delusion that she ever had a real vagina.
Ted isn’t questioning whether she’s a cis woman or a dude. He’s calling into question the validity of transness to begin with. When he asks what genitals she has, he wants to know if she’s post op or pre op.
Everyone shuns Ted because they reject his questioning that trans women might not be women, and they accept Claire as a “woman.” But again, that has nothing to do with passing.
If the comic was about her passing, then the room would be full of people who had never been told that Claire was trans and who believed she was cis, and they would all be very confused by Ted’s behavior and wouldn’t have any idea what the fuck he was talking about.
