There probably should be a difference; laws have been passed to protect the status of trans people, so if a sexual identity is the same as a fetish, then everything from pedophilia, bestiality, and necrophilia are just as valid as claiming to be heterosexual or homosexual. I was going to argue for a difference, but right now I can't think of a meaningful line where a fetish ends and an identity begins. The concept of a fetish originally was an object that held power, and if an activity holds power over a person, then that fetish is something the person can't really control, thus it's a part of their identity... So I guess there is no difference. Unless you can think of one.
