See, just because you are born in the US and have a US passport doesn't mean you necessarily identify as American.
You could decide that instead you identify with the culture of your immigrant parents, or maybe you strongly identify with the people you grew up around who weren't Americans. Or maybe your people were never accepted so you don't accept the US either.
Same with sex and gender. You have a strong and persistent feeling that your body is wrong for who you are in that you cannot accept the gender your sex suggests? Then you are trans. And you feel as a man in a female body, a woman in a male body, or maybe something else.
And even independent of that, there are women born with no vagina, or no uterus, or an underdeveloped reproductive system. Nevertheless, they know themselves to be women, and telling them they aren't is very hurtful.
Just like men with small dicks are still men, and you are being super offensive if you declare them not men.
That's because their gender identities are independent of the development of their bodies. And although they suffer from the state of their body, they suffer far more if they are ridiculed and denied their identity.
