When an especially powerful Ayakashi named Shirogane comes after Suzu to eat her and take her power, Matsuri intervenes using an ancient binding scroll that his family has been slowly charging up for decades to use against especially hard to beat spirits. While being sucked up by the scrolls, Shirogane's last spiteful act is to cockblock Matsuri because he can see that these kids are totally going to bone, and he's a petty bastard. So he hits Matsuri with a spell thats turns him into a girl.
Shirogane, as it happens, was powerful enough that a little bit of him is left over even after the scroll sucks up all it can, so now you have this fat white housecat with a bone to pick with the main characters. Shirogane is playing 4D chess the whole time, and he is both the only one that can remove the curse AND he doesn't have enough power to remove the curse unless they release him from the scroll and let him return to his full power. So he's constantly pestering the main duo to try and get them to return him to full power or steal the scroll and do it himself. Meanwhile, Suzu is trying to turn him into a legit ally so giving him his powers back isn't immediately a disaster. Matsuri, for the most part, doesn't seem to give a fuck about the fact that he's a girl now. He cares more about the fact that his arms and legs are shorter and his center of gravity is different, throwing off his ninja training, than the fact that he has boobs now. Suzu however wants matsuri back to normal as fast as possible because, and I literally cannot stress this enough, *she wants that dick*.
This is how the manga starts, but the table has been flipped on the plot like twice since then.
