Anyways to answer OPs question, nothing exactly has "gone wrong" with the series on a permanent level. Season 6 I wasn't fond of due to varying reasons, the setting change hurt the series and the characters all felt off, not to mention the obnoxiously abrupt ending of the season to the point where I didn't even realize it ended until like a month later. But S7 managed to win me back and really excelled beyond the season prior, I felt it was the best the show had been since S3.
I think the delays between seasons, while obnoxious as all hell, really benefit the show as it gives Publick and Hammer plenty of time to make sure it's as good as possible without worrying about deadlines or getting burned out.
It's just one of those prices we have to pay for good content I guess.
