For example, He used to be rose quarts/pink diamond but is now Steven, he struggles with trying to get people to call him by his trans name, and the climax of his story arc involves his family accepting him as his trans name, as Steven.
Of course, most stories about self esteem can be applied to anyone, it's just that the gay people who made this put a gay bent to it.
That said, I think what's literally taking place dampens both the message and it's impact by a fair amount. Having the genocidal, controlling, dogmatic, Nazi mom break down so quickly reeks of being rushed.
