It turns out Billy doesn't transform into Captain Marvel, he swaps places with him, and whoever's not currently active is kept in a stasis chamber in the Rock of Eternity. Sivana discovers this and has his children create a distraction to attract Captain Marvel, leaving him free to sneak into the Rock and steal Billy's body.

Captain Marvel doesn't understand why he can't turn back into Billy, and eventually tracks down Sivana in his lab, where Billy's body has been dissected.

Cap stitches Billy's body back together and revives him frankenstein-style with the thunderbolt, and everything is fine.
