I've been thinking about that and there's a few things I came up with.
1.Someone she knew got hurt by a villain they arrested who escaped.
2.She fails to save someone. This can tie into both her feelings of inadequacy and her feeling that they need to do more than just arrest villains to send a message.
I don't wanna go full "somebody died" but definitely someone got badly injured or she just failed in some spectacular way.
