Read again what you wrote and try to make sense of it. Even if you try, I don't see how that relates to Ishigami. If she wanted approval she could have received right BEFORE having sex. Those are literally the less direct of the consequences. The one that was directly hurt was Ishigami, and Aka made him to just say "nah gurl I'm good". I'm not saying he has to go full incel but can't it be at least akward?, it was shown at the beginning of the arc that Tsubame deeply regretted what she had done (dark face n' shit) but right after it turns out Ishigami was barely affected by it. Now she barely has to do anything more than probably say sorry and go out with Ishi. . Even if what she did is bad it was just a mistake from her. People do that, it happens.
What doesn't happen is that the person you directly hurt just tells you it was nothing. What doesn't happen is that after doing shit like that things go back to the status quo (Ishi wants to love, Tsu rejects but kinda likes him). It was the perfect situation for a redemption arc, where a character has to deal with fucking it up too hard and reconstructing what it broke.
Kaguya-sama always had a kinda realistic approach, but this just seems forced so that the public doesn't hate Tsubame.
